SEAFARER’S VISA REQUIREMENTS
·Duly accomplished application form
·One passport sized photo, pasted on the application form
·Valid passport & photocopy at first 2 pages
·Medical Certificate (original & photocopy) TAKEN STIRCTLY WITHIN 2 MONTHS
·Employment Contract with Shipping or Manning Agency and copy of the POE Addendum/ Standard Terms & Conditions
·POEA Contract (Original & Photocopy)
·Argentine Immigrations Digital Authorization to apply for visa
·Letter of Guarantee from Filipino Agent
·Letter of Request from Filipino Agents Stating the Following:
oName of Applicant/Position
oMode of Transportation and itinerary
oName of Vessel/Country/Flag Carrier
oAgent & Contact Person with complete address and contact nos. in Argentina
·Air Ticket (Original and Photocopy) or Booking Certificate
·If going to other countries, their visas should be taken first
Processing Time: AT LEAST FIVE (5) WORKING DAYS
Filing Hours: 9:00 am –11:00am (Monday –Friday)
Releasing Hours: 2:00 pm –4:00 pm (Monday –Friday)
TOURIST VISA REQUIREMENTS
·Duly accomplished application form
·One passport sized photo, pasted on the application form
·Valid passport & photocopy of the information pages
·Letter of request from applicant stating the reason for the visit
·Original Certificate of Employment indicating salary and duration of leave. If owner, SEC or
Mayor’s Permit (Originaland Photocopy)
·Valid NBI Clearance (Original and Photocopy)
·Medical Certificate with Doctor’s License Number
·Original Bank Statement &/or Passbook (Original and Photocopy)
·Latest Income Tax Return (Original and Photocopy)
·Confirmed Hotel Bookings/Hotel vouchers
·Confirmed Air ticket or Booking Certificate
·If going to other countries their visas should be taken first
·For Foreign Nationals, ACR/ICR
