 Original and Photocopy of NOC (No Objection Certificates)
 Original and Photocopy of Medical Certificate by GAMCA
 Original and Photocopy of NBI Clearance (Authenticated by DFA)
 1 pc. 2x2 Colored Picture
 Visa Stamping Fee: Php4,000 – including authentication of NBI
