1. Telex coming from Immigration of Libya for approval indicating the name and passport
number
2. Passport with Arabic translation
3. Round-trip ticket with confirmed flight
4. One (1) passport size colored picture
5. Marriage Contract (if married)
6. NBI
7. Passport copy with Residence Visa of the Sponsor
8. Birth Certificate (for minor)
9. Employment Certificate/Contract of the sponsor signed by the Employer
10. Affidavit Support
11. Medical Certificate authenticated by DOH and DFA
12. Visa application form and Curriculum Vitae / Bio-Data
