Requirements:

* Mayor’s permit or Barangay Permit to operate business

* SEC Registration

* Articles of Incorporation

* Board Resolution

* Secretary’s Certificate to Borrow and Authorized Signatory for corporation

* Latest 3 months bank statements with negotiated checks

* With co-borrower (should be one of the incorporators) and co-maker

* 2x2 pictures

* 2 valid IDs

* Proof of billing (electric, water)

*Proof of residence (telephone, cable, etc)

*Photos of business

Co-maker/ Co-borrower Requirements:

*2x2 picture

*2 valid ids

*latest proof of billing (electric, water)

*documented source of income

*must have a residential telephone or cellphone
