          5. Passport
          6. 2 photos (passport size)
          7. Personal Appearance
     If married to a Pakistani:
          1. Letter of invitation from Pakistan (Attested by the Ministry of Foreign Affairs, Islamabad)
          2. Photocopy of Husband’s passport & National Identification Card
          3. Marriage Contract (Original & Photocopy)
              (Attested by the Ministry of Foreign Affairs, Islamabad / D.F.A., Manila)
          4. NBI Clearance (Authenticated by the D.F.A.)
          5. Visa Application Form
          6. 2 photos (passport size)
          7. Personal Appearance
     If a child of a Pakistani:
          1. Photocopy of Parent’s passport and National Identification Card
          2. Birth Certificate (Original and Photocopy)
          3. Photocopy of parent’s marriage certificate
          4. 2 photos (passport size)

     Business:
          1. Letter of invitation from the company in Pakistan
          2. Letter of request from the applicant’s firm in the Philippines / Certificate of Employment
          3. NBI Clearance (Authenticated by the D.F.A.)

     Work:
          1. Permission from the Ministry of Interior, Government of Pakistan, Islamabad.
          2. Philippine Overseas Employment Agency (POEA) clearance or OEC
